CASE MANAGEMENT SERVICES
InspirationField provides Case Management services to all eligible persons living in Otero, Crowley, and Western Bent Counties. Case Management is often the first point of contact for individuals and families seeking services. Case Managers will provide support to individuals, their guardians and families to promote independence and inclusion in their communities.
Your Case Manager will be your resource for information and will assist you through systems to help you plan for your future and find the services you need. The Case Manager will determine eligibility for services. Once eligibility is determined, the Case Manager will develop an Individual Plan based on person-centered planning. Then, the Interdisciplinary Team will implement the Individual Plan.
Individuals who are age five or older must have a developmental disability that is manifested prior to age 22. Developmental disability means a cognitive impairment attributable to mental retardation or similar conditions that causes significant skill deficits in at least two of the follow areas: communication, self-care, home living, social skills, community access, self-direction, health/safety, functional academics, leisure and work.
